NEW PASSPORT

  1. Fresh Form No. 1 to be filled and signed.
  2. Four color photos with white background passport size.
  3. Copy of Xth class certificate or birth certificate duly self attested.
  4. Copy of two residence proofs duly self attested are required like:
    1. MTNL/BSNL telephone bill.
    2. Election Identity Card.
    3. Electricity bill.
    4. House tax receipts.
    5. Rent Agreement/Lease-deed.
    6. Driving Licence.
    7. Gas connection receipt.
    8. Pass-book of any nationalized bank.
