PROCEDURE FOR LOST PASSPORT

1. Fresh Form No. 1 to be filled and signed.
2. Four colour photos with white background passport size.
3. Self attested copies of old passport first four pages and last four pages if available.
4. Original copy of FIR.
5. Affidavit.
6. Covering letter addressed to Passport Officer.
7. Copy of two residence proofs duly self attested are required like:
   
   1. MTNL/BSNL telephone bill.
   2. Election Identity Card.
   3. Electricity bill.
   4. House tax receipts.
   5. Rent Agreement/Lease-deed.
   6. Driving Licence.
   7. Gas connection receipt.
   8. Pass-book of any nationalized bank.
