CHANGE OF NAME AFTER MARRIAGE

1. Fresh Form No. 1 to be filled and signed.
2. Four colour photos with white background passport size.
3. Copy of marriage certificate duly self attested or Annexure-D duly notarized.
4. Copy of proof of residence self or your husband like:
   
   1. MTNL/BSNL telephone bill.
   2. Election Identity Card.
   3. Electricity bill.
   4. House tax receipts.
   5. Rent Agreement/Lease-deed.
   6. Driving Licence.
   7. Gas connection receipt.
   8. Pass-book of any nationalized bank.
      
5. Copy of passport of your husband first four pages and last four pages duly self attested.
